San Andreas Emeregency Service - Los Santos Medical Division - Medical Account Application

Hello, citizens of Los Santos.

In order to perform health checks, and to be healed more effectively (if needed), you will need to apply for a Medical Account.
Its simple - It is basically another ID card, with your personal and health information, wich will help the Doctor that is healing you to know who you are, and how to heal you.

All you need to do, is to fill the following form and send it to any hospital by mail, or give it to the Director by hand.
